Building independence, connection, and confidence beyond the centre

 

Through our Community Support provision, we aim to create a network of care, inclusion, and opportunity for children and young people with disabilities across Gloucestershire.

We work closely with each young person to understand their needs and goals, offering tailored support packages that encourage growth, confidence, and life skills in real-world environments.

 

🧭 Our Purpose

Our Community Support is built around three core aims:

  • To help children and young people gain independence, life skills, and social confidence

  • To support re-engagement with the wider community through meaningful activities

  • To provide families and carers with flexible short breaks and reassurance

Each package of support is personalised, with input from families, carers, and professionals. Outcomes might include managing money, using public transport, attending appointments, developing friendships, or simply building resilience and emotional wellbeing.

It is a flexible, responsive service that adapts to each family’s routine. It is also very different to our Day Centre support.

✨ What makes Community Support different from our Day Centre?

While our Day Centre offers a set environment full of structured group activities and on-site facilities, Community Support is designed to happen out in the world, or sometimes at home.

It might look like:

  • Helping a young person travel independently

  • Supporting them to attend a dentist appointment

  • Cooking a meal together at home

  • Going swimming, shopping, or catching a train

  • Building confidence and routines in everyday environments

Community Support is all about helping young people thrive in real-life settings, while giving families short breaks that fit around their own daily lives.

🧩 Tailored Support in the Community

We support young people across Gloucestershire with a wide range of needs, from low to high complexity. Some need practical help, others need emotional regulation and behavioural support. Most need a bit of both.

Our team takes the time to understand:

  • What works well

  • What doesn’t

  • And how we can adapt our approach to help each young person succeed

We also work alongside professionals such as the PBS team to make sure our care is consistent, informed, and joined up.

🌍 What Community Support Can Look Like

We offer a wide range of services based on what each young person needs. This includes:

  • 2:1, 1:1, or small group sessions

  • Support attending appointments

  • Life skills coaching such as using transport, going shopping, managing money, preparing meals, and more

  • Emotional wellbeing support, helping young people build confidence, routines, and positive coping strategies

  • Social engagement and local adventures, including trips to:

    • Bristol Zoo

    • The beach

    • Bournstream

    • Cattle Country

    • Catch a Smile

    • Tenpin Bowling

    • Swimming

    • Wake The Tiger

    • …and many more

🤝 Who is it for?

Our Community Support service is open to disabled children and young people who would benefit from structured, supportive time in the community or home.

We accept referrals for individuals:

  • With an EHCP or identified support needs

  • Aged 5 to 25

  • Living in Gloucestershire or nearby areas

  • Who need low, medium, or high levels of support

We accept referrals from parents, carers, schools, social workers, and local authorities.

Once we receive a referral, we will get in touch to talk through the best next steps and begin creating a support plan built around the individual needs of your young person.

🕒 Flexible Support, When You Need It

Our Community Support provision is designed to fit around the real lives of the families we work with.

We are a 24-hour service, which means support can be arranged at any time. This includes daytime, evenings, weekends, or even overnight, depending on what works best for your child or young person.

Whether it is help during school hours, support attending an evening activity, or care during the night so families can rest, we are here to make it happen.

We build our sessions around what each family needs, because no two lives are the same and support should reflect that.
